- Child information record - you may drop off a hard copy
- Copy of your child's birth certificate - you may drop off a hard copy
- Health appraisal form for any new student or any students entering Preschool, Kindergarten, or 7th grade - you may drop off a hard copy. *Students cannot enter school without proper immunization records or waiver on file.
